In consideration of being allowed to participate in the event or activity referenced above, I acknowledge, appreciate, and agree that:

1) The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,

2) I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,

3) I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,

4) I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS the Releasees, their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("RELEASEES"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR

OTHERWISE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

For good and valuable consideration, the receipt of which is hereby acknowledged, I hereby grant Youth Development Now / OnBall Sports permission to use my likeness in a photograph/video in any and all of its marketing, including but not limited to all printed and digital publications, websites and social media. I understand and agree that any photograph or video using my likeness will become property of UBC and will not be returned.

I acknowledge that since my participation with this event is voluntary, I will receive no financial compensation.

I hereby irrevocably authorize Youth Development Now / OnBall Sports to edit, alter, copy, exhibit, publish or distribute this photo for purposes of publicizing their programs or for any other related, lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising out of, or related to, the use of the photograph.

I hereby hold harmless and release and forever discharge UBC from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

Refund Policy

OnBall Sports and Utah Top 50 will refund 100% of registration fees if registration is canceled more than 7 days prior to the registered event.

50% of the registration fee will be refunded if registration is canceled 3-7 days prior to the registered event unless the participant chooses to convert the registration fee to event credit. See Below.

No refunds will be given if registration is canceled less than 3 days prior to the registered event.

Event Credit

Canceled registrations are eligible for event credit to another OnBall Sports event in lieu of a payment refund.

100% of the registration fee can be converted to event credit rather with a cancellation with 7 days of the registered event.
